Cerebral aneurysmal childhood arteriopathy: a rare complication of pediatric HIV.

The left internal carotid artery appears dilated and ectatic suggestive of fusiform aneurysm of left internal carotid artery. Fusiform dilatation of vessels of circle of Willis to form large aneurysms, termed “Cerebral Aneurysmal Childhood Arteriopathy”, is an exceedingly rare complication of pediatric HIV(1). We report one such case in a 12-year-old-child with WHO clinicalstage-4 HIV disease, who was admitted with complaints of headache and right hemiparesis.

Human immunodeficiency virus (HIV)-associated cerebral aneurysmal arteriopathy is described in the pediatric medical literature and features diffuse fusiform aneurysms of the arteries of the circle of Willis. A 34 day-old girl infant was admitted for poor feeding and cholestasis. She had a bulging fontanelle, with no evidence of intracranial infection or hemorrhage. Investigations demonstrated that she had galactosemia. Computed tomographic scans demonstrated the presence of diffuse cerebral edema. After treatment the edema resolved.
